This Consumer Health Data Privacy Policy explains how Smithey Longevity Group LLC ("SLG," "we," "us") collects, uses, shares, and protects consumer health data. It is provided to comply with Washington State's My Health My Data Act (MHMDA) and applies to the consumer health data we process about the people we work with. It supplements any other terms or privacy notices we provide.

SLG provides educational and analytical wellness services. We are not a healthcare provider, and nothing we do is medical advice, diagnosis, or treatment. Please consult a licensed clinician for medical concerns.

This policy covers that data. We collect it only with your consent and use it to provide you with personalized, educational wellness analysis and recommendations.

Identified vs. de-identified use

To provide your personalized service, we necessarily use data that identifies you. Separately, and only with your optional consent, we may use de-identified data (with your name and identifying details removed) to improve our methods and for aggregate analysis. We do not attempt to re-identify de-identified data, and we require anyone who receives it to do the same.

You may withdraw any consent at any time, as described below.

We keep your consumer health data only as long as needed to provide your service and for legitimate, disclosed purposes, and then we delete or de-identify it. You can request deletion at any time (see your rights below).

No system is perfectly secure, but we work to protect your data and to limit what we collect to what we need.

Where we de-identify data, we take reasonable measures to ensure it cannot be linked back to you, we publicly commit not to re-identify it, and we contractually require recipients to honor the same commitment.

We will verify your request and respond within 45 days as required by law (we aim to act sooner), and we will not discriminate against you for exercising your rights. If we deny a request, you may appeal by replying to our response.

We do not sell consumer health data, and we will not do so without the separate, valid authorization that the law requires.

12. Children

Our services are for adults. We do not knowingly collect consumer health data from anyone under 18.

13. Changes to this policy

We may update this policy. We will post the updated version here with a new effective date, and where required we will obtain your consent before using previously collected data for materially new purposes.
